New Patient Information

At Evergreen Dental, we respect the value of your time and will make every effort to honor all time commitments with a minimum waiting time. Please extend the same courtesy to us by arriving promptly for your appointment. Please contact us at least 24 hours in advance if you are unable to keep a scheduled appointment. There will be a charge for repeated broken appointments.

Payment Options

The ability to pay can worry many dental patients. We understand your concern and can offer a number of payment options. We will discuss the payment plan best suited for you in privacy and assist you in making all the arrangements.

  • If you have dental insurance, we file claims as a courtesy for our patients. However, any coverage denials will become the patient's responsibility. We will assist you to the best of our abilities in your appeal process.
  • Payment is due at each appointment as well as fees not covered by insurance. Cash, checks and most major credit cards are accepted.
